Step-by-Step Guide

Step 1: Visit the City Health Office

    • Go to the Outpatient Department (OPD)
    • Midwife on duty will ask your reason for consultation
    • Your details will be recorded in the Individual Treatment Record (ITR)

Step 2: Patient Assessment

    • Midwife takes your medical history and vital signs
    • You’ll be referred to the physician on duty
      ⏱️ 2 minutes

Step 3: Medical Examination

    • Physician examines you
    • Prescribes medicine and gives advice
    • Refers you to the pharmacy for free medicine (if available)
    • If hospitalization is needed, a referral form will be issued
      ⏱️ 10 minutes
