  • BLS Training Certificate and ID from Naga City Community First Responder Program Management Office (NCCFRPMO)
  • Community Tax Certificate from City Treasury / Barangay Hall
  • Barangay Clearance from the barangay where the applicant resides

Submit the application forms from the Sangguniang Panlungsod Office Task Force
