ABOUT THE SERVICE

A Certificate of Indigency is issued for clients who will avail of the services of the Naga City Solo Parents Affairs Office (NCSPAO).

Who Can Apply

Individuals who meet any of the following:

  • An unmarried parent who chooses to raise their child
  • Parent left solo due to:
    • Death of spouse
    • Spouse in detention
    • Spouse’s physical/mental incapacity
    • Legal or de facto separation
    • Annulment/nullity of marriage
    • Abandonment
  • A woman who gave birth due to rape
  • Any person solely providing parental care
  • Family member acting as head of household

Requirements

For New Applicants:

  • 1 pc 1×1 photo
  • 1 pc 2×2 photo
  • Filled-out application form

  • Filled-out ID form (All forms available at NCSPAO)

 

For Renewal:

  • Membership Renewal Form
  • Member validation
